Comunicado de prensa Comité de vinculación y campos clínicos

En el contexto actual de pandemia en nuestro país, las carreras de la salud no han dejado de trabajar de manera conjunta en la vinculación con los campos clínicos, es por esta razón, que la Escuela de Obstetricia y Puericultura de la Universidad de Valparaíso para mantener y fortalecer la unión entre campos clínicos y la escuela, se ha creado por resolución Exenta N° 83457, el Comité de vinculación con el medio y campo clínico.

El Comité se encuentra integrado por los docentes de las distintas sedes: Reñaca; Paula Eherenfeld, Soledad Herrera, David San Martín y San Felipe: Leslie Olivares, Juan Pablo Rojas. Además, está conformado por representantes de empleadores, titulados y de la unidad de capacitación y campos clínicos externos:

Su objetivo es crear un espacio permanente que permita fortalecer la relación con distintos actores del medio social, especialmente con aquellos que se vinculan con el quehacer académico y estudiantil de la Escuela Obstetricia y Puericultura de la Universidad de Valparaíso.

Dicho comité tiene por funciones, las siguientes:

Mantener un vínculo de comunicación con aquellos actores que se relacionan con el quehacer académico y estudiantil.
Generar espacios de diálogo entre la comunidad de la Escuela de Obstetricia y Puericultura y los actores externos.
Establecer propuestas de mejoras entre los representantes sociales pertenecientes al comité.
Visibilizar el proyecto formativo de la Escuela Obstetricia y Puericultura de la Universidad de Valparaíso.
Coordinarse con la Unidad de Campos Clínicos de la Vicerrectoría Académica y con el Representante de la Facultad de Medicina en el Consejo de Vínculos con el Medio de la Universidad de Valparaíso.

Los integrantes de la escuela realizarán reuniones sistemáticas 1 veces por semestre y de forma anual con los integrantes externos. Esperamos que esta nueva instancia sea provechosa para el futuro de nuestra Escuela y genere una comunicación directa entre estos estamentos.

El miércoles 01 de septiembre se realizó el conversatorio con estudiantes de sexto básico del Seminario San Rafael de Valparaíso cuyo objetivo fue Entregar conocimientos y experiencias a los/las/es estudiantes relacionados a los cambios asociados durante la adolescencia. La actividad estuvo a cargo del prof. Danilo Zamorano y con participación de las estudiantes María José Zuñiga y Daniela Becerra. Asistieron aproximadamente 44 estudiantes los/las/les estuvieron muy atentos/as a la exposición y posterior aclaración de dudas. Agradecemos a la profesora Marianela Perez, quien fue la gestora de esta actividad.

La actividad educativa se denomino «Libertad Reproductiva y Sexual», la cual se realizó en Colegio Sagrados Corazones – Padres Franceses de la ciudad de Viña del Mar , la cual estuvo orientada a estudiantes de III año Medio, donde se conversaron las temáticas de Salud Sexual y Reproductiva entre otras. La actividad tuvo una modalidad híbrida, con un grupo de estudiantes de forma presencial y la otra parte vía virtual.

La actividad estuvo marcada por una alta participación de estudiantes (60 en total) y con apoyo de la profesora Patricia Rodríguez, Claudia Vilches y Danilo Zamorano, Coordinador de la Unidad de Vinculación con el Medio y Extensión.

Además se contó con la presencia de la Sra. Ximena Ampuero, Encargada Unidad de Promoción y Relaciones Públicas de nuestra Universidad, quien a su vez gestiono la participación de dos promotores/as, Angel Herrera y Josefa Beltrami, quiene expusieron sobre nuestra Universidad y generaron un stand de consultas con folletería de todas las Carreras.

Con más de 100 participantes entre docentes y estudiantes, se realizó la Jornada de Matronería de este año en honor a la celebración del día de la Matrona y del Matrón

Con más de 100 participantes entre docentes y estudiantes, se realizó la Jornada de Matronería de este año en honor a la celebración del día de la matrona y del matron, actividad que estuvo marcada por la reflexión, el autocuidado y el fortalecimiento de las relaciones de nuestra Escuela.

Tanto los/las estudiantes como los/las docentes presentaron videos sobre los desafíos, aprendizajes y anécdotas vividas en el proceso de docencia durante la pandemia. Además se contó co la presencia de dos profesionales de salud mental, las psicólogas Karen Cofré y Leslie Carrasco, quienes a través de sus presentaciones hicieron reflexionar sobre todo lo aprendido durante esta crisis sanitaria.

SIMUV de la Facultad de Medicina aplica esta innovadora metodología, que recrea casos reales.

Un innovador método formativo en modalidad remota, basado en la representación de casos clínicos que acontecen en escenarios o ambientes hospitalarios virtuales y en la interacción en línea -por medio de una pantalla- de estudiantes con especialistas, pacientes y otras identidades diseñadas digitalmente implementó el Centro de Simulación Clínica (SIMUV) de la Facultad de Medicina de la Universidad de Valparaíso.

La iniciativa busca que los futuros titulados de las carreras de Medicina, Enfermería, Fonoaudiología, Kinesiología, Obstetricia y Puericultura, Tecnología Médica y otras vinculadas al área de la salud aprendan a tomar decisiones trascendentes en tiempo real y que adquieran y practiquen las habilidades inherentes a sus respectivas disciplinas y al trabajo en equipo, al tener que realizar un procedimiento, determinar un diagnóstico o enfrentar una urgencia.

“Esta metodología, que bautizamos como E-casos, la comenzamos a aplicar el año pasado ante la necesidad de sustituir algunas actividades presenciales en nuestras sedes de Reñaca y San Felipe, debido a la pandemia de COVID-19. Pero al poco tiempo nos dimos cuenta de su utilidad y de las potencialidades que tiene para los procesos de enseñanza-aprendizaje de nuestros futuros profesionales, tanto en lo didáctico como en lo dinámico, ya que permite la transmisión sincrónica y simultánea a varios cursos. Además, fomenta el desarrollo de la creatividad, del pensamiento crítico y la participación de los estudiantes frente a la consulta de un paciente o una jefatura, una atención de urgencia o tener que resolver algún problema puntual. Por todo ello, este año, con el respaldo de distintos equipos docentes, decidimos incorporarla en forma permanente en algunas asignaturas y programas de pregrado”, explica la coordinadora del SIMUV, Macarena Ugarte.

¿Cómo funciona?

La formación basada en escenarios virtuales es uno de los métodos que considera la simulación clínica y es utilizada ampliamente por diversas instituciones a nivel mundial.

En Chile su desarrollo aún es incipiente y está acotado a algunas iniciativas específicas, entre las cuales destaca el proyecto FONIS destinado a diagnosticar el Alzheimer en el que trabaja otra unidad de la Facultad de Medicina de la Universidad de Valparaíso, el Centro de Investigación del Desarrollo en Cognición y Lenguaje, junto a los servicios de Neurología de los hospitales Carlos van Buren y Gustavo Fricke.

“Tomando en cuenta ésta y otras experiencias, y ante la necesidad de generar alternativas de educación a distancia para nuestros estudiantes, debido a la pandemia, nos dimos a la tarea de desarrollar nosotros mismos una plataforma propia que hiciera posible incorporar esta metodología”, precisa el técnico en simulación clínica y programador del SIMUV, Fabián Vera.

El acceso a esta aplicación se realiza por medio de un enlace de internet que conduce a una interfaz en la que cada estudiante es recibido por una identidad virtual (médico, matrona, enfermera o kinesiólogo, entre otros) que puede estar situada dentro de un quirófano, pabellón, sala de emergencias o frente a un monitor de signos vitales y que mediante la activación de su voz formula preguntas o solicita a su interlocutor tomar una decisión en base a una serie de alternativas que se despliegan en la pantalla y que son factibles de ser seleccionadas.

“También es posible representar de igual manera a un paciente, un familiar o profesor y generar múltiples respuestas y escenarios. Por ahora, dado que el desarrollo de todo esto toma tiempo y es necesario capacitar a los docentes, hemos acotado su aplicación a algunas asignaturas y carreras, pero la idea es avanzar en su utilización en el pregrado, en la medida en que los docentes -de quienes hemos tenido una buena recepción- tengan la capacidad de respuesta necesaria”, asegura Vera.

Ventajas

La matrona y docente de la Escuela de Obstetricia y Puericultura UV Greisny Azócar, que imparte la asignatura práctica virtual de Neonatología en tercer año, y que estuvo entre las primeras en asumir el desafío, valora de gran manera esta modalidad de simulación a distancia.

“Esta es una innovación que sin duda requiere de práctica de parte de todos los involucrados, porque nos obliga a aprender bien una metodología y a dominar una serie de aplicaciones tecnológicas con las que hoy recién nos estamos familiarizando, pero que tienen enormes ventajas y potencialidades, considerando que a futuro la telesalud y las prestaciones a distancia serán algo habitual”, afirma la académica.

Entre sus fortalezas, la profesional destaca la entrega de contenidos en forma mucho más dinámica y el hecho de que favorece el acompañamiento y la guía del docente a cargo.

“Asimismo, facilita la inmersión en el caso y la obtención de respuestas inmediatas y en diferentes contextos. Por ejemplo, frente a una hipotético diagnóstico, reacción o cambio en algún síntoma o signo vital. En general, los estudiantes se han adaptado bien, pero para algunos esta metodología ha resultado compleja, porque además requiere de una buena conexión a internet. No obstante, su respuesta ha sido satisfactoria. De hecho, realizamos una encuesta y el noventa por ciento de mis alumnos señaló que esta modalidad es útil y les ha ayudado en el aprendizaje”, asegura la profesora Azócar.

Al respecto, Benjamín Vargas, quien cursa tercer año de Obstetricia y Puericultura, sostiene que esta opción es más didáctica y atractiva que la simulación a distancia tradicional.

“Hay más variables en juego, voces, acciones, posibilidades de intervenir… Si bien tiene muchas cosas que mejorar, me parece que es un gran avance respecto de lo que estábamos haciendo, que consistía más que todo en observar lo que otros hacen o exponen. Incluso, esta metodología podría ser implementada como parte de las prácticas profesionales”, asegura el alumno.

Una visión similar expresa su compañera en la asignatura práctica virtual de Neonatología Sofía Lazo: “Comparada con otras experiencias de simulación a distancia, los E-casos son muchos más enriquecedores y efectivos a la hora de situarse en un escenario clínico a través de la pantalla. Si bien hay cosas de la simulación clínica presencial y de la práctica presencial que son imposibles de sustituir, este tipo de realidad virtual, que incluye la interacción con avatar, es sin duda una opción que contribuye a generar experiencias mucho más enriquecedoras y efectivas”.

Cada 31 de Agosto se celebra un día más de la matrona y el matrón, este año nuestra profesión cumple 187 años y lo queremos conmemorar recordando cómo nos hemos adaptado a los cambios demográficos, epidemiológicos y socioculturales de nuestro país. Es así como desde el 2020 estamos viviendo una pandemia mundial y la atención en Salud ha cambiado, sin embargo matronas y matrones seguimos realizando nuestras labores, adecuándonos al contexto actual, sin olvidar la mirada humanizada de la atención. Este mes, la Escuela de Obstetricia y Puericultura de la Universidad de valparaíso ha preparado diversas actividades para celebrar nuestro día, entre ellas la jornada “Matronería en tiempos de COVID: Experiencia desde la Neonatología” con la destacada participación de la Matrona gestora de procesos Neonatales en el Hospital Barros Luco Trudo Luz María Román, quien tiene una vasta experiencia en el área de Neonatología, Lactancia Materna y docencia Universitaria.

Para la Red de Escuelas y Carreras de Obstetricia de Chile, de la cual nuestra escuela forma parte, es muy preocupante la formación integral de sus estudiantes, ya que hay aprendizajes que no se pueden adquirir por vía virtual, simulación ni con pacientes estandarizados. Por este motivo, algunas Escuelas se encuentran con movilizaciones estudiantiles que buscan también visibilizar la difícil situación que están vivenciando en el proceso docente. Desde inicios del año 2020 hemos vivido como país una crisis sanitaria sin precedentes que ha puesto al límite las capacidades humanas de todo el personal que trabaja en salud y la población en general.

Los esfuerzos realizados para combatir la pandemia han dejado en un segundo orden, de manera entendible hasta cierto punto, algunas cosas, sin embargo, la falta de educación para aquellas y aquellos estudiantes de carreras relacionadas al área de la salud, está llegando a un punto crítico, ya que desde hace más de un año y medio que hay una ausencia crítica de prácticas clínicas para estudiantes en niveles intermedios. Esta realidad pone en riesgo el modelo actual de la formación de nuevas/os egresadas/os para este 2021 y años posteriores.

Para la estudiante de Universidad San Sebastián, Javiera Salinas Valdés (22), cree que no se están haciendo los esfuerzos suficientes “esto no es un juego, pronto seremos profesionales de la salud y es algo serio, vamos a trabajar con personas”, cuenta en conversación con El Mostrador Braga.

“Hay cosas en nuestra carrera que no son intercambiables por horas de simulación o de telemedicina como el simple hecho de aprender a interactuar con las y los pacientes. Nuestra carrera se desarrolla de una manera integral, por lo tanto no vemos solo la parte científica, también nos desenvolvemos con nuestras usuarias en su parte emocional y eso no te lo entrega una práctica online”, relata.

Empeorando aún más la situación, para Javiera y muchas estudiantes más, han tenido que llegar al punto de buscar empleos de Tens para “aprender a tratar con pacientes, aprender prácticas básicas relacionadas con la salud, porque lamentablemente de los 4 años que llevamos estudiando, en nuestra generación no ha tenido ninguna práctica (…) Es frustrante, es desgastador, es triste levantarse todos los días con la preocupación y poca esperanza de que si nos van a mandar o no algún día a nuestras prácticas”.

Y es que desde el año 2020, no solo las y los estudiantes de la Carrera de Obstetricia y Puericultura (Neonatología) se han visto afectadas/os en sus aprendizajes por la imposibilidad de ingresar a los centros de salud de la red de servicios hospitalarios y de atención primaria para la realización de prácticas clínicas, sino que muchos otros futuros profesionales han perdido una parte fundamental para su formación y desarrollo óptimo de competencias y habilidades propias de su profesión.

En este sentido, las instituciones formadoras, han realizado múltiples adaptaciones metodológicas y didácticas para seguir llevando a cabo los compromisos asumidos en sus perfiles de egreso y mallas curriculares, a la espera de la apertura de los centros de salud. No obstante, “sabemos que nuestros esfuerzos no son suficientes para eliminar la brecha que implica la ausencia parcial, y en algunos casos casi total, del contacto directo con las/os usuarias/os del sistema de salud”, explican a través de una carta, la Red de Escuelas y Carreras de Obstetricia de Chile.

“Nos preocupa la formación integral de nuestras/os estudiantes, sabemos que hay aprendizajes que no se pueden adquirir por vía virtual, simulación e incluso con pacientes estandarizados. Por este motivo, algunas Escuelas se encuentran en movilización estudiantil de actividades académicas, reflejando la difícil situación que estamos vivenciando en el proceso docente”, agregan.

Es por ello que ante este panorama, solicitan encarecidamente realizar todos los esfuerzos necesarios para la apertura de las prácticas clínicas en los distintos servicios de salud y municipios para estudiantes curriculares. “El futuro de los profesionales de salud está en peligro”, aseveran.

La formación profesional requiere de habilidades y competencias que sólo se pueden adquirir en el trabajo presencial con personas y con integración en los equipos de salud.

Las soluciones académicas ya se han realizado, ahora se requiere el apoyo para dar una respuesta a esta necesidad, que no solo afecta a las/os estudiantes de la Obstetricia y otras carreras de salud, sino que a toda la población, garantizando la atención de calidad en Salud Sexual y Reproductiva centrada en la persona, también les preocupa la reducción del personal de recambio necesario en condiciones en que los equipos se encuentran desgastados por la demanda que ha implicado la contingencia sanitaria.

“Volvemos a hacer un especial llamado al Ministerio de Salud y a la Subsecretaría de Redes Asistenciales a dar una pronta y certera respuesta, más explícita y/o un criterio más uniforme para los distintos servicios de salud, que permita el ingreso de estudiantes curriculares a sus prácticas profesionales, considerando las normas, precauciones y estándares propios del quehacer en salud. Estamos seguras/os que como escuelas de Obstetricia seremos de gran ayuda en estos momentos de dificultad”, determinan.

Para lograr lo anterior, es que están dispuestas/os a establecer vías de comunicación expeditas, para poder organizar junto a las autoridades locales, este reintegro tan esperado a los campos clínicos.

El martes 13/07 se publicarón las nuevas rotaciones de 2° a 4° año, en donde se incorporan para 4°año a las actividades practicas virtuales y talleres SIMUV, las pasantias practicas intrahospiralarias desde el 26 de julio y a contar de septiembre las pasantías de Salud Familiar. Recalcamos el gran esfuerzo que ha significado en terminos de gestión el poder concretizar dichas pasantías.

Durante hoy miércoles 14/07 se realizará la primera reunión del comité de buenas prácticas clínicas.

El jueves se realizará la primera reunión con el grupo de 6 estudiantes que asumurán desde julio como ayudantes SIMUV. Esto incluye una beca de apoyo para los estudiantes que realizaran la actividad.

En el transcurso esta semana seguiremos desarrollando de manera exitosa nuestros talleres personalizados SIMUV Obstetricia, únicos en la zona, completando así 90 sesiones desde inicio de Julio, en que nuestras alumnas y alumnos han podido desarrollar sus competencias técnicas por medio de la aplicación de distintos procedimientos a nivel Gineco- Obstétrico, como también de la Neonatología.

Nos alegra la alta participación que hemos tenido durante el desarrollo de nuestros talleres, como también los buenos comentarios y el apoyo por parte de nuestro Estudiantado, los cuales son los actores relevantes dentro de este Proceso Formativo. También agradecemos la entrega de parte de nuestro Equipo Docente quienes han dado todo su esfuerzo para que nuestros estudiantes tengan una formación de calidad, de manera colaborativa, amena y sobresaliente.

Como Equipo de Obstetricia debemos destacar que la pandemia no ha sido un impedimento para lograr nuestros últimos objetivos, ya que se ha podido aplicar de manera eficiente y efectiva cada uno de los talleres con grupos reducidos, cumpliendo con todas las normativas vigentes para la protección de nuestra Comunidad UV y por sobretodo con un alto estándar de calidad basado en la empatía, convivencia y respeto entre Docentes y el Estudiantado.

En el día de hoy Martes 06/07 continuamos con los talleres integrados para estudiantes de 4to año para reforzar la técnica de especuloscopia y toma de examen papanicolau, respetando el protocolo COVID-19 establecido por nuestra Universidad.

Con más de 50 participantes se realizó la Primera Jornada de Investigación en Salud, diversidad sexual y género: una aproximación desde la Matronería, con motivo de la conmemoración del Mes del Orgullo. La actividad organizada por la Escuela de Obstetricia y Puericultura de la Universidad de Valparaíso y Fundación Iguales se realizó el día miércoles 24 de Junio a través de la plataforma Zoom. Dentro de los/las/les expositores/as destaca la Doctora © Cynthia Vergara de la Universidad Austral, profesor Danilo Zamorano y la estudiante Estefany Vera, quienes presentaron una panorámica sobre la situación actual en salud e investigaciones relacionadas al colectivo LGBTIQ.
